Lyndene celebrates outstanding success

We’re delighted to share some success news from our Lyndene colleagues, who would like to celebrate the incredible work of Community Support Worker, Jennifer Scholfield.

Jennifer has achieved fantastic success in supporting a family within the community, meeting and even exceeding the agreed referral outcomes. Through her dedication, she introduced tailored visual supports for use at home, helping to create a more structured and supportive environment for the family.

Going above and beyond expectations, Jennifer’s impact has been truly life changing. As a result of her work, one of the sisters has overcome previous barriers to engaging in personal care and this has been an enormous step forward in terms of her independence and dignity. A milestone that was not originally part of the local authority’s outlined outcomes, making this achievement even more remarkable.

Jennifer’s colleague shares,

“Jenny has done some amazing work with NY. While others have visited the home, it’s mostly been Jenny. Grandma reports that NY has been engaging in personal care and, during my visit, she was also using the communication book.”

This wonderful feedback highlights the difference she makes every day. As her colleagues so rightly put it:

“It’s life-changing work that she has done.”

A huge well done to Jennifer for her passion and the positive impact she continues to have on the lives of the people she supports.
