Personalised budgets

 

In partnership with service users, families and local authorities the Together Trust has a range of services that can be accessed via personal budgets.

We have a great deal of experience in defining service specifications funded by the Independent Living Fund and continuing health funding and Local Authority funding. All services provided through our domiciliary agency have been rated at the highest 3 star ‘Excellent’ level by the Care Quality Commission.

We provide a personalised service by embracing the core values of choice and control, value for money, innovation and creativity, whilst balancing safety and well-being. Our aim is to foster and develop services that enrich people’s lives, their sense of well-being and self-esteem, along with community access, independence and full citizenship.

Our responses to personal needs and preferences are driven by person-centred approaches to achieve the vision of success, as defined by the service user and those closest to them.

We map our service success against the ‘Putting People First’ framework, which includes:

  • Health and well-being
  • Economic well-being
  • Making a contribution
  • Safety from harm, neglect and harassment
  • Choice and control
  • Maintain maximum respect and dignity.

All supported living and community outreach services aim to help people participate and engage in their local communities, whilst increasing opportunities for social and work activities. Support workers provide individual and group support, including:

  • Supporting service users to actively manage their own homes and perform household tasks as independently as possible
  • Making and keeping appointments related to ordinary living, health and well-being
  • Pursuing personal development activities, such as work or volunteering
  • Accessing and taking part in social and leisure activities
  • Initiating and consolidating personal interests and peer relationships.

The Together Trust seeks to make information available in a user-friendly way and to avoid people needing to approach multiple agencies in order to find the exact support and services they need.

We will help you design your own solutions and work with you to provide creative choices to meet your needs.

We aim to cost services realistically and fairly, whilst providing support from well trained and well supported workers. The Together Trust’s finance systems have been adapted over recent years to manage individual budgets, and to support you in planning and making payments.
