Short, medium and long-term care

 

The Together Trust offers short, medium and long-term care for children, young people and vulnerable adults up to the age of 30. 

All our CQC and Ofsted registered homes are based in the North of England and have been awarded ‘Good’ or ‘Outstanding’ at their last inspection.

Solution focused services

With an excellent reputation for partnership working, all our services for young people and adults offer truly welcoming, friendly and safe environments. Our short, medium and long-term care homes in the North West of England provide:

  • Support from teams of highly trained, experienced staff. Training is provided in PROACT SCIPr-UK®, PECS based communication, Makaton and autism specific training where relevant
  • Person centred planning – linking to all aspects of support, social integration and learning, we work closely with schools and colleges
  • Access to Child and Youth Care Education Services (CYCES) for young people with Behaviour, Emotional and Social Difficulties (BESD)
  • Proven success in achieving positive outcomes in preparing children/sibling groups for all adoption, fostering or re-unification with their birth families
  • Assistance with transition and direct work packages where appropriate
  • Support from our own clinical psychologist
  • Our staff are trained to support those with physical and/or learning disabilities and complex health needs.

We also provide activity holidays, flexible outreach, and community and home services for young people and vulnerable adults across the North of England. We have a number of service user participation forums, groups and activities to promote friendships and enhance service developments.

We are committed to the provision of quality care and improved outcomes for some of the most disadvantaged children, young people and vulnerable adults in society.
