Supported tenancies

 

Together Trust supported tenancies provide home living and community access support to vulnerable adults who have a physical/learning disability, autism or complex health needs.

Services are delivered to individuals who are tenants in their own or shared home, rather than those who live in a residential care home. The aim of the service is to enable people to live full and enriched lives within their local community, to experience a broad range of relationships and build support networks.

The success of our 24/7 support has been achieved by creating small teams of workers with specific interests and skills that are matched to the individual to provide continuity. This approach, combined with our commitment and implementation of person-centred planning has allowed us to develop a tenancy model that is fit to meet a range of emerging complex needs. As with all of our services, 24/7 support packages are developed with service users and their families at the centre wherever possible.

As a result of our unannounced CQC inspection in September 2009, the service was given the highest award - 3 Stars/‘Excellent’ rating. We feel that this has been achieved by our re-enablement approach implicit to support plans, excellent and experienced staff and a clear vision for standards and expectations in service delivery.

  • Healthy living and exercise

Support packages are tailor-made to the unique needs of each service user.
