Outreach and Family Support

 

The Together Trust outreach and family support services are for children, young people and vulnerable adults with complex needs including autism spectrum conditions (ASC), mild to severe learning and/or physical disabilities.

Our services aim to meet the needs of individuals with complex health needs and challenging behaviours. We deliver holistic care through proven approaches to person-centred planning.

Our outreach service enables people to attend local and community activities fully supported by qualified staff. This enables parents and carers to take a break, safe in the knowledge that the service user will receive expert care and stimulation.

We are committed to the provision of quality care and improving outcomes. We pride ourselves on the excellent links and relationships we form with the service user receiving our support, their families and the communities in which they live. 

We provide choice and control in all aspects of support to service users.
