Home based support

The Together Trust provides flexible services to support individuals with complex needs, including autism, mild to severe learning and/or physical disabilities, complex health needs and challenging behaviours.

We provide holistic care for individuals through person-centred approaches/planning.

Our home services are designed to be supportive and unobtrusive to the family. We offer a range of services that include sitting services during the day, evening or overnight and weekends. This service is invaluable for families with young people or adults who have complex health needs or palliative care requirements. 

We also provide family based carers, offering home assistance, providing that extra pair of trusted hands.

We promote choice and control in all aspects of support offered to individuals.

The white paper ‘Our Health, Our Care, Our Say’ formed the vision for the development of a personalised approach. Our services are based on this vision and include creative and innovative care packages for individuals in supported tenancies and within family homes.
