Domiciliary services and 24 hour support

The Together Trust provides a wide range of support services including specialised short breaks, flexible outreach, community and home services for children and adults.

Our outreach, community and home services offer:

• Specialised support for children, young people and adults with complex needs - working in partnership with families/carers, often within their home environments
• A comprehensive assessment process, including risk assessments and the development of support plans
• Holiday, weekend and overnight breaks for individuals and families
• Day services
• Short break fostering schemes
• Community support – offering fun and enjoyment to help develop life skills and ongoing learning opportunities, promoting independence and empowerment. An emphasis is placed on being healthy, keeping fit, community involvement and enhancing peer/sibling relationships
• Family and carer support services, including sitting services
• A holistic service with continuity of approach for those accessing the Together Trust education and/or residential/supported living services
• Individualised packages to support transitions; identifying future needs and requirements based on person centred planning
• Personal assistant support to promote accessibility within education and the workplace.
